Habilitation Aide

Description of Position:

Provide hands on individualized assistance to consumers with developmental disabilities to improve the quality of life for each of them.  Act as a support person for the consumers and their family members, while providing assistance in skill building in activities of daily living.  Provide guidance to foster improvement in social skills.  Ensure the integration and inclusion of the consumers by assisting them in becoming productive, contributing members of their community.

Essential Duties & Responsibilities:  

  • At all times, serve as an advocate for the consumer with whom you work.  The consumer’s health, safety, welfare and human dignity is your highest priority.
  • Provide direct care and assistance to consumers with developmental disabilities and their families. Work hours may include weekdays, evenings and weekends as designated by the needs of the consumer, and as established by program schedule.
  • Accompany consumer(s) to all medical, educational, employment and legal appointments. Assist with giving and receiving of all pertinent information as it relates to the appointment. Inquire about follow-up appointments and procedures as needed.  Complete follow-up documentation as required.
  • Effectively utilize written and oral communication. Work cooperatively with consumer, family members, Department Head, Waiver Supervisor, Service Coordinators, Habilitation Aides, clerical staff, Tribal programs and other professional staff and agencies to constructively resolve matters that may arise.
  • Interview consumers to discuss their service needs, which may include the following: personal, family, household, financial, employment, physical, social, legal, recreational, mental health and medical concerns.
  • Assist Waiver Supervisor and Service Coordinators with the develoopment and implementation of consumer's Individualized Service Plan (ISP). Assist in the development of Day Habilitation, Residential Habilitation or Respite Plans.
  • As assigned, investigate and gather background information such as, medical, psychological, social and educational information pertaining to the consumer’s. This includes making the appropriate telephone calls, letter writing, communicating with the individual, communicating with other agencies and programs.  Share this information with Supervisor, Service Coordinators, Habilitation Aides, and other personnel as needed.
  • Participate and actively contribute to the consumer’s annual, semi-annual and emergency Individualized Service Plan Meetings.
  • Assist consumers in achieving their individual goals by adhering to the designated Individualized Service Plan.  This could include, but is not limited to the following tasks:  assistance with budgeting, paying bills, grocery/personal shopping, transportation, recreational outings, life planning, scheduling of appointments as needed, researching and obtaining resource information, assistance with personal problem solving, medication administration and/or monitoring, assistance with personal hygiene, meal planning, cooking, housekeeping, parenting, social interaction, and other daily living skills as needed.
  • Utilize a team approach and positive work habits.  Attend and actively participate in scheduled staff planning meetings.
  • Completion of tasks assigned by Waiver Supervisor or Department Head to include linkage and referral of consumers to other agencies, tribal programs and community resources.
  • Ensure regular entries in consumer’s progress notes once service has been delivered.  Complete notes in a timely manner and by established guidelines.
  • Completion of other required documentation as assigned.
  • Gain knowledge of and continued learning of the policies and procedures of the St. Regis Mohawk Tribe, Family Support, HCBS Waiver and other related programs.
  • Attend, actively participate in, and complete required training as directed.
  • Assist with other duties necessary within the programs such as dishwashing, van cleaning, cooking, shopping, collecting receipts and participating in gatherings and parties.

Qualifications Required:

High School Diploma or GED.  Education or experience working with developmentally disabled is preferred.  Must be 21 years of age or older with a valid New York State driver’s license and a clean driving record is required.

DRUG TESTING POLICY

The successful applicant for employment must submit to a drug test for illegal substances prior to employment being confirmed.  The testing is performed at the laboratory at the Alice Hyde Hospital in Malone, NY.   Test results are normally received in three to five days from the date of the test.  In the case of a positive result, the candidate is not eligible for employment with the St. Regis Mohawk Tribe.  Employment may not begin until this process has been completed.
