General Description:
The Admissions Officer/RN has two primary functions. As a member of a multidisciplinary addiction treatment team that guides the overall care, support, and family engagement process at the Partridge House, a 16-bed American Indian inpatient program, is responsible for overall nursing care of patients, including assessments, planning of care, teaching of daily living skills and evaluation of expected outcomes; administration of medications; reviewing test results; communicating with physicians, nurse practitioners and other healthcare team members to ensure the highest quality patient care; and referral to aftercare services. As Admissions Officer, facilitates the admission process which requires regular contact with referring sources, processing admission packets and ensuring that criteria for admission is met and scheduling intake. The Admissions Officer/RN functions as a member of the administrative team responsible for policies, procedures, and quality assurance. Additional responsibilities include but are not limited to the provision of program (e.g., health lectures), carrying a case load (1-2), transportation to medical appointments, and medical triage on weekends and after business hours (on-call). This position requires valid NYS Registered Professional Nurse licensure, in good standing. Certification in addiction nursing (i.e., CARN) or addiction counseling (i.e., CASAC) is a plus.
Essential Duties & Responsibilities:
- Functions as a member of the Partridge House Inpatient Program administrative team
- Compiles, supervises and documents all necessary information during the referral process
- Reviews medical histories, medication lists, and lab work to determine the Level of Care (ASAM Criteria) for all prospective clients
- Provides consultation on all prospective referrals to the Inpatient Coordinator and/or Clinical Director to ensure appropriate Level of Care determination, prior to admission
- Schedules, coordinates, and otherwise facilitates all admission dates of new clients
- Performs Initial Medical Assessments for admissions and validates appropriate Level of Care at time of intake
- Reviews and records all medication into the Medical Administration Records (MARS)
- Performs and documents all scheduled and random urine screens and breathalyzers of clients
- Develops, with the Clinical Director, medical policies and procedures for the delivery of all medical services
- Coordinates and schedules all Complete Physical Exams (CPX) with the Family Nurse Practitioner and/or medical staff at the SRMT Health Clinic
- On-call status: responsible for Medical Triage services after business hours and on the weekends
- Schedules and manages the daily medically-related activities of all clients
- Completes inventory of all outdated over-the-counter medications, checks the First Aid Kit and AED machine monthly for restocking and ordering of batteries and otherwise assures adequacy of medications, supplies and equipment
- Provides transportation and medical case management for all on-campus medical appointments
- Reviews the Medical Administration Records (MARS) for any medication errors
- Facilitates Quarterly Assurance Medical Inventory conducted by the SRMT Pharmacy Department
- Provides educational lectures on Public Health topics (e.g., smoking, diabetes, medical aspects of addiction, more)
- Responsible for the review of Medical Incident Reports
- Responsible for participating in supervision sessions
- Participates as a member of the Partridge House Inpatient Program Multidisciplinary Team in reviewing intake assessments, treatment plans, progress, and discharge/aftercare plans
- Participates in morning report and weekly case consultation meetings
- Responsible for completing education and training related to addiction in general and addiction nursing specifically. Certification in addiction and/or addiction nursing is encouraged, e.g., CASAC, CARN)
- Represents the Partridge House Inpatient Program in a knowledgeable and professional manner at outreach and community activities conferences, and during routine conversations with referral sources
- Maintains all licensing/credentialing requirements and standards (i.e., in good standing)
- Performs all duties in an ethical manner and complies with all ethical standards (e.g., professional, tribe)
- Maintains all confidentiality standards and regulations (e.g., federal, state, tribe)
Qualifications:
- High School Diploma or GED
- NYS license to practice as a Registered Professional Nurse, in good standing
- Minimum of two (2) years experience working in the chemical dependency field
- Experienced in working with American Indians in a behavioral health treatment Or Indigenous Healing environment; knowledge of Mohawk language and traditions is an asset
- Excellent communication skills, both orally and written
- Ability to perform professional caliber work with minimum supervision
- Leadership skills
- Physically able to carry out the duties of the position
- Minimum of three (3) years sobriety (if in recovery from addiction)
- Must have and maintain a valid NYS driver’s license
- Successful candidate must submit to a urine drug screen
- Native American Preference
