OUTREACH NURSING DEPARTMENT The Outreach Nursing Department provides various services to the community. Care management/case management for the Elders in Akwesasne require an assessment of each individual to establish their needs. Some of these services provided through the Outreach Department are: skilled nursing, home care referrals, medication management, foot care, education and several health screenings including audiometric and vision. The area of Maternal Child Health takes care of tracking the immunization rates and ensuring that infants and young children keep their appointments and update their immunization records on a regular basis. An early Risk Reduction Class as well as the Childbirth Education Class is available for all pre-natal. The Community Health Nurses with the assistance of the Community Health Representatives provide glucose and blood pressure monitoring in the community as well as in the home settings. Transportation is provided for the elderly and disadvantaged with a 24-hour notification before the appointment date.
