HOME AND COMMUNITY BASED WAIVER SERVICES

In 1996, the Home and Community Based Waiver Services program began with services that offered Day Habilitation and Residential Habilitation. Individuals must be eligible for Medicaid. Staffing assistance is provided for each individual as needed. Day Habilitation is any activity, which takes place within the community as designated by the persons individualized service plan goals. Activities can include, but are not limited to shopping, bill paying, banking, transportation to personal and medical appointments, recreation and socialization activities. Residential Habilitation is any activity, which takes place within the home as designated by the persons individualized service plan goals. Activities include but are not limited to budgeting, meal preparation, house cleaning and laundry.
