GENERAL SUMMARY: The Child and Youth Behavioral Health Care Coordinator is a member of the Mental Health clinical team that guides the care and support of clients being served in the Mental Health clinic (including school-based satellites). Under the direction of the Child and Youth Mental Health Clinic Coordinator, the Care Coordinator will assist the Mental Health therapists (including school-based) with meeting the client's needs by providing care coordination and advocacy for clients.
ESSENTIAL DUTIES AND RESPONSIBILITIES the Care Coordinator will work within guidelines of the Saint Regis Mohawk Tribe Health Services policies and procedures, including Mental Health, and under standards of care according to New York State Office of Mental Health and Substance Abuse Mental Health Services Administration (SAMHSA) while engaged in the following:
- Exhibit a friendly and courteous manner when dealing with clients, fellow employees and associates.
- The Care Coordinator will assist with improving Mental Health service delivery with schools and youth serving agencies by completing screenings, coordinating care, and facilitating referrals.
- The Care Coordinator will provide advocacy, attend meetings, coordinate and facilitate wrap around meetings for children, youth and their families/caregivers to meet needs, improve functioning and improve social and emotional health.
- Work as part of the behavioral health team to carry out client’s treatment plans.
- Coordinate service delivery with other agencies, schools and programs within Health Services.
- Taking and triaging referrals - gathering from the referral source the emergent/emergency nature of the referred persons need for treatment, screening for behavioral health disorders, and triaging for care needed.
- Assist Clinic and School-based therapists with care coordination, case management and engagement with children, youth and their families/caregivers.
- Linking - The process of referring or transferring a client to supports and services of their choice and need.
- Monitoring - Observation to assure continuity of service in accordance with the client’s treatment plan.
- Case-specific Advocacy - Interceding on behalf of a client and their family to assure access to services required in the individual treatment plan.
- Organize and facilitate wraparound/care coordination meetings when needed.
- Outreach and Screening: Provide mental health information and screenings at Head Start, local schools, local colleges, Akwesasne Girls and Boys Club, and community events.
- Providing outreach and engagement of clients into services.
- Establishing working relationships with youth and young adult serving agencies.
- Participate and assist with grant activities, data collection, planning meetings and evaluation.
- Documenting client contact according to NYSOMH standards of care and for purposes of the Akwesasne Youth Suicide Prevention Project.
- Provide transportation of clients as needed.
- Conduct clerical grant duties such as filing, scheduling, onboarding, compiling statistics
- Other duties as assigned within scope of work as a Child/Youth Care Coordinator.

EDUCATION and/or EXPERIENCE: Associate degree in the areas of psychology, human services, counseling or other applicable major, and two-years of experience working with children and youth in behavioral health, school, social services or other child/youth serving agencies. Interest in obtaining additional training/education in Behavioral Health, suicide prevention, intervention and postvention.
OTHER SKILLS and ABILITIES: Strong oral and written communication skills required. Must be organized and have good computer skills including, Word, and ability to navigate various scheduling and electronic health record programs. Openness in working with traditional/spiritual approaches regarding behavioral health issues. Ability to do presentations, represent the mental health program and connect with people. Motivated, caring and compassionate, committed to suicide and substance use prevention, reducing impact of stigma and increasing help seeking behavior. Able to follow NYS Justice Center Code of Conduct and be a positive role model for Akwesasne youth and their families. Familiarity with Akwesasne Community.
REASONING ABILITY: Maturity and ability to handle delicate role issues and to respond to crisis. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
Possession of a valid State Driver License in accordance with Fleet Policy.

The Saint Regis Mohawk Tribe Health Services has adopted a “Vaccination Policy” that requires ONLY Health Service employees to be “Fully Vaccinated” against the Coronavirus. The Vaccine requirement DOES NOT apply to positions that are not Health Services employees.
