Registered Nurse/Behavioral Health Care Coordinator

SUMMARY: Under the direction of the Psychiatric Nurse Practitioner, the RN/Care Coordinator will assist the Mental Health therapists, Psychiatrist, Psychiatric Nurse Practitioner, and the Outpatient Alcohol and Chemical Dependency Program staff with meeting the client's needs

 

The RN/Care Coordinator will work within the guidelines of the Saint Regis Mohawk Tribe Health Services policies and procedures, including Mental Health and A/CDP, and under standards of care according to the New York State Office of Mental Health and New York State Office of Substance Abuse and Alcohol Services.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

· Exhibit a friendly and courteous manner when dealing with clients, fellow employees, and associates.

· Work as part of the behavioral health team to carry out client’s treatment plans.

· Coordinate service delivery with other agencies, schools, and programs within Health Services.

· Taking and triaging referrals - gathering from the referral source the emergent/emergency nature of the referred persons need for treatment, screening for behavioral health disorders, and triaging for care needed.

· Provide injections to clients receiving injectable psychotropic or Medication Assisted Treatment.

· Assist Clinic and School-based therapists with care coordination, case management, and engagement with adults, youth, and families.

· Linking - The process of referring or transferring a client to supports and services of their choice and need, especially with physical health services.

· Monitoring - Observation to assure continuity of service in accordance with the patient's treatment plan, health monitoring, support medication management, and health promotion.

· Case-specific Advocacy - Interceding on behalf of a client and their family to ensure access to services required in the individual treatment plan, especially with Medical Clinic, Outreach, LGHP, and pharmacy.

· Assist with health monitoring and management of behavioral health clients, provide physical health assessments, screening, and outreach in the community.

· Facilitate wraparound/care coordination meetings when needed.

· Outreach and Screening: Provide mental health information and screenings at Head Start, local schools, local colleges, Akwesasne Girls and Boys Club, and community events.

· Providing outreach and engagement of clients into services.

· Establishing working relationships with youth and young adult serving agencies.

· Participate and assist with grant activities, planning meetings, and evaluation.

· Assisting clients with health monitoring and medication issues, including obtaining prior authorization for medications.

· Reviewing health history forms and making recommendations for physical health follow-up.

· Performing health assessments with ACDP outpatient clients.

· Assisting with Continuing Quality Improvement project and reporting requirements.

· Documenting client contact in all the electronic health records that Health Services uses (RPMS Behavioral Health electronic record, EHR, and Script Pro) according to New York State Office of Mental Health and Office of Alcohol Substance Abuse Services standards of care.

· Assisting with nursing duties at Partridge House when needed.

· Other duties as assigned within the scope of work as an RN/Behavioral Health Care Coordinator.

 

SUPERVISORY RESPONSIBILITIES: There are no supervisor requirements for this position,

Licensed registered nurse.

 

EDUCATION and/or EXPERIENCE:

· Licensed as a Registered Nurse in New York State. AND

· 1 year experience as a Registered Nurse in Behavioral Health, inpatient or outpatient settings

Preferred

· 1 year medical-surgical hospital experience as a Registered Nurse

 

 

OTHER SKILLS and ABILITIES: Familiarity with Akwesasne Community. Interest in obtaining additional training/education in Behavioral Health. Strong oral and written communication skills required. Must have excellent computer skills, including Word and Excel, and experience with varying databases. Computer testing in the applicable skills may be a requirement during the interview process. Openness in working with traditional/spiritual approaches regarding behavioral health issues. Ability to do presentations, represent the mental health program, and connect with people. Motivated, caring, and compassionate, committed to suicide and substance use prevention, reducing the impact of stigma and increasing help-seeking behavior. CPR certification and physically able to carry out the duties of the position.

 

REASONING ABILITY: Maturity and ability to handle delicate role issues and to respond to crisis. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
