SUMMARY: Under the direction of the Waiver Manager, the Day Program Supervisor is responsible for providing oversight and supervision for the Disabilities Services Day Program. The Day Program Supervisor will be responsible for managing the program by defining the structure and protocols of the program, as well as ensuring that documentation is provided for billing purposes. Will establish cooperative working relationships with social service agencies and vocational rehabilitation services and will establish a good working rapport with family and community members. Will also be responsible for hiring and supervising of employees, scheduling and employee evaluations, and for ensuring that quality services are provided for the individuals that participate in the Day Program.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: 

 

  • At all times, serve as an advocate for the individuals for whom you work. The individual’s health, safety, welfare, and human dignity are the highest priority.
  • Supervise a team of Direct Support Professionals and support staff, ensuring the quality of care and the carryover of Individualized Service Plan Valued outcomes. In particular, ensure that all Day, Community, and Residential Habilitation plans are completed and documented for and completed in the required time allotment for submission for billing.
  • Investigate and gather information to improve the quality and efficiency of the program.
  • Monitor and provide direction to staff through actively engaging in daily activities. It is expected the Day Program Supervisor will make random spot checks to ensure staff compliance with scheduled activities.
  • Utilize a team approach at all times when engaged in activities with the other staff.
  • Establish a good rapport with individuals, advocates, family members, and guardians to ensure individual satisfaction and quality programming.
  • Complete employee schedules and provide relief staff as needed.  Will be required to fill in if other staff is unavailable.
  • Provide direct supervision to the staff employed in the day programs and Residential Habilitation program.
  • Implement a productive working environment that generates high staff morale through effective staff achievement and performance.
  • Complete employee evaluations annually and after the initial probationary period.
  • Ensure that adequate/mandatory training is provided to employees to enable them better to provide quality services to the individuals.
  • Provide support, assistance, and oversight to Intellectual Disabled individuals participating in the Day program services. Assist with providing and clarifying individual information to DSPs and support staff and other concerned parties
  • Ensure that employees record progress notes relating to individual Residential Habilitation plans and Day Habilitation plans. Ensure that an adequate number of notes are recorded for billing purposes. Ensure that services are delivered within the required time frame and documented as such.  Review all notes completed by DSP’s for accuracy and completeness. Submit billing reference sheet to Biller for processing for payment on a monthly basis.
  • Track the number of units of service utilized by the Day Program. Monitor budget expenditures to ensure that resources are properly allocated, and maximize billing to ensure the overall financial well-being of the program.
  • Work cooperatively with the individual, family members (if requested), and other program personnel as needed to guarantee that each individual receives all services he/she is entitled to. Take an active role in all Life Plan Meetings.
  • Work hours will be Monday through Friday 8am-4pm. Hours may vary during the week, depending on the needs of the program. On-call status is required throughout the month for all of the programs. Re: On call policy  
  • Identify areas of need within the community and engage in new and innovative initiatives to expand the scope and encourage the growth of the Day Program.

SUPERVISORY RESPONSIBILITIES: The Day Program Supervisor is responsible for the supervision of the Day Program as directed by the Waiver Manager. The Day Program Supervisor is additionally responsible for initiating purchases of supplies and equipment, working with the Waiver Manager and other staff on assigned duties and special projects, exercising authority in problems/issues relating to staff and programming of the program, and may be assigned special projects periodically by the Waiver Manager. The Day Program Supervisor supervises and evaluates staff performance and maintains Tribal policy and Department directives,

 

QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • Successfully obtain all required certifications to provide initial and annual training.

 

EDUCATION and/or EXPERIENCE: 

  • Associate’s Degree in Human Services or a related field AND
    • Three (3) years of documented work experience in the field of developmental disabilities, mental health, or related field 

OR 

  • Equivalent experience/education 

Preferred

  • One (1) year of supervisory experience. 

 

OTHER SKILLS and ABILITIES: A strong understanding of computer use, particularly with regards to electronic documentation. Strong oral and written communication skills required. Must have excellent computer skills, including Word, Excel, and experience with varying databases. Computer testing in the applicable skills may be a requirement during the interview process.

 

This position mandates employees to report to or remain at work in the event of an emergency, crisis situation, or staff shortage until relieved by their immediate supervisor.

 

REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

 

Must have and maintain a valid driver’s license in accordance with the Saint Regis Mohawk Tribe’s Fleet Policy.
