SUMMARY:

Home Health Care Supervisor oversees a staff of aides who provide support to patients in their homes with personal care and housekeeping. The supervisor orients and trains staff in departmental and organizational policies and procedures; schedules aides, directs their work assignments, and monitors their quality of care. The supervisor ensures that aides have gained a basic understanding of hands-on personal care, patient safety, and interacting with elderly and dementia patients.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Recruit, interview and select employees, complete hiring process. Train new employees on job duties and company policies.
  • Design, implement, and evaluate monthly staff training or arrange for outside agency to provide.
  • Provide annual employee job performance reviews with home health aides. Identify issues in a timely manner and address causes and corrective actions.
  • Maintain employee records of trainings and in-services.
  • Ensure hours worked are accurate in ADP; approve/deny requests for time off in order to maintain adequate workforce.
  • Prepare and issue work schedules to Aides on a weekly basis, update Aides and patients daily related to changes in schedule.
  • Conduct home visits in coordination with Case Managing Nurse for initial assessment and care plan development. Review home care services agreement, safe patient handling program, and services offered by Chronic Care Department.
  • Ensure Care Plan and hours allotted for patients are provided as written.
  • Identify actual and potential health problems in the home and in the community.
  • Promote education and safety in the home and community through education on injury prevention, health hazards including smoking, alcohol use, poor eating habits, poor hygiene, or any medical problems.
  • Conduct Patient satisfaction surveys or performance measurement criteria to further the development of programs.
  • Participate in screening patients, case management and record keeping.
  • Collaborate with other members of the professional health care team as well as outside agencies related to patients care.
  • Facilitate good communication between the home health aides and the nursing team to reach the patient’s care objectives. Update nurses on changes in health conditions of home care patients.
  • Document patient care in electronic health record in a timely manner.
  • Assist in managing medical supplies and equipment inventory. Ensure that necessary supplies are stocked and available for patient care, and report any shortages to the appropriate personnel.
  • Complete quarterly and annual reports on clients served and hours of service provided, staffing levels, describe any other program areas of concern.
  • Assist in the development of program policy and procedures.
  • Complete all duties of a Home Health Aide when needed.
  • Other duties as assigned

 

MANDATED REPORTER: Must become a mandated reporter prior to having contact with children in a job-related capacity and maintain that status throughout employment. this includes completing any necessary training and adhering to all reporting obligations as defined by applicable laws and tribal policies.

SUPERVISORY RESPONSIBILITIES: The Home Health Care Supervisor is responsible for the supervision of the Home Health Aides.

 

EDUCATION AND/OR EXPERIENCE:

  • Licensed Practical Nurse (LPN) from an accredited nursing program, Current NY State License AND
    • One-year related experience in chronic disease case management PREFERRED.

OR

  • Personal support worker or home health aide certificate AND
    • At least 2 years of experience.

 

OTHER SKILLS AND ABILITIES: Exhibit a friendly and courteous manner when dealing with patients, their caregivers, and fellow associates. Maturity and ability to handle delicate patient medical and mental health issues, family involvement and the ability to respond to crisis. Strong oral and written communication skills required. Must have excellent computer skills, including Microsoft Office, Adobe, and be able to learn electronic health record database. Knowledge of Mohawk language and traditions is considered an asset. Must be physically able to carry out the functions of the position.

 

Must have and maintain a valid driver’s license in accordance with the Saint Regis Mohawk Tribe’s Fleet Policy. Successful candidate must pass a drug test and background check prior to employment.
