Summary
The Internist provides comprehensive primary and specialty medical care to adult patients, focusing on the prevention, diagnosis, and treatment of acute and chronic illnesses. The Internist is responsible for performing detailed assessments, developing treatment plans, managing complex medical conditions, and coordinating care across specialties when necessary. Scope includes chronic disease management (such as diabetes, hypertension, cardiovascular disease, COPD), preventive health services, routine physical exams, medication management, diagnostic interpretation, and patient education.
Essential Duties & Responsibilities
- Provide comprehensive adult medical care including assessment, diagnosis, treatment, and follow-up.
- Manage chronic health conditions using evidence-based clinical guidelines.
- Conduct routine physical examinations, wellness visits, preventative screenings, and chronic care appointments.
- Order, interpret, and follow up on diagnostic tests (lab work, imaging, specialty evaluations).
- Prescribe and manage medications in accordance with medical standards and regulatory requirements.
- Develop individualized treatment plans and ensure clear communication with patients and families.
- Coordinate care with specialists, behavioral health, pharmacy, nursing, and other team members to ensure continuity of care.
- Provide patient education and counseling on disease prevention, lifestyle modification, and self-management.
- Participate in care team huddles, case reviews, and multidisciplinary planning.
- Maintain accurate, timely documentation in the electronic health record (EHR).
- Participate in quality assurance, performance improvement, and clinical protocol development.
- Follow all clinic policies, medical bylaws, infection control standards, and Tribal/Federal/State regulations.
- Support culturally sensitive and community-centered care practices in alignment with organizational mission and values.
- Perform other clinical duties as assigned to support patient care and clinic operations.
Mandated Reporter: Must become a mandated reporter prior to having contact with children in a job-related capacity and maintain that status throughout employment. This includes completing any necessary training and adhering to all reporting obligations as defined by applicable laws and tribal policies.
SUPERVISORY RESPONSIBILITIES: There are no supervisor requirements for this position.
MINIMUM QUALIFICATIONS:
- Medical degree (MD or DO) from an accredited institution And
- Completion of an accredited residency in Internal Medicine and board eligibility or certification in Internal Medicine and active New York State medical license or eligibility to obtain one.
AND
- 3 years working experience in internal medicine in a hospital or outpatient setting.
- Experience working with Native American populations preferred.
OTHER SKILLS and ABILITIES: BLS and ACLS certifications or obtain within 90 days. Experience in hypertension management and chronic disease care. Strong clinical reasoning, diagnostic, and communication skills. Commitment to culturally respectful care and serving Indigenous communities.
