SUMMARY:  The Nurse Case Manager is responsible for providing comprehensive nursing care, diabetes education, and case management services to patients living with diabetes. This role emphasizes patient-centered care, chronic disease management, and interdisciplinary collaboration to support improved health outcomes. The Nurse Case Manager under the direction of the Nursing Supervisor will collaborate with all members of the health care team to ensure high quality patient care.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Uphold the value and integrity of the Saint Regis Mohawk Tribe’s Mission Statement.
  • Comply with all organizational policies to include Ethics and Compliance Policy, Customer Care/HIPAA standards in all patient interactions.
  • Deliver age-specific and diagnosis-specific nursing care tailored to individual needs.
  • Apply clinical knowledge and critical thinking to support optimal patient outcomes.
  • Provide direct nursing care, including initial patient assessments, vitals, and health education.
  • Provide education to patients and families in clinical, home and community settings.
  • Provide Continuous Glucose Monitoring education, set up and analysis.
  • Lead patient education initiatives and group classes as needed.
  • Assist primary care providers with chart prep, intake, patient rooming, and documentation.
  • Perform EKGs and Point of Care tests as ordered for provider review.
  • Administer vaccine and any medications per established protocols ensuring complete documentation.
  • Perform comprehensive annual diabetic foot exams in accordance with Indian Health Services and American Diabetes Association standards of care.
  • Safely and effectively use all medical equipment within their scope of practice. Maintain competency with current equipment and acquire proficiency with any new devices introduced into the clinical setting.
  • Collect, label, and handle all specimens accurately, ensuring proper documentation and timely transport to the lab to support quality processing and optimal patient care.
  • Maintain awareness of safety and infection control procedures, demonstrating a knowledge of appropriate protective measures by consistently utilizing required personal protective equipment to ensure a safe care environment.
  • Adhere to emergency protocols and respond effectively to urgent situations.
  • Provide complete and accurate documentation per protocol utilizing effective communication techniques in all systems used.
  • Coordinate and ensure completion of consults, referrals, and prior authorizations to include obtaining records and follow-up.
  • Registered Nurse may be required to float to other departments or care areas as needed to support staffing requirements and ensure continuity of patient care within Health Services/Diabetes Center.
  • Demonstrate strong teamwork and initiative by assisting with tasks and patient care needs proactively, contributing to a supportive work environment.

 

MANDATED REPORTER: Must become a mandated reporter prior to having contact with children in a job-related capacity and maintain that status throughout employment. This includes completing any necessary training and adhering to all reporting obligations as defined by applicable laws and tribal policies.

 

Supervisory Responsibilities:   There are no supervisor requirements for this position.

 

QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE:  

  • Registered Nurse from an accredited nursing program with a current NY State License AND
    • Two (2) years of hospital experience as a registered nurse

OR

  • Registered Nurse from an accredited nursing program with a current NYS state license, AND
    • Two (2) years of hospital like experience AND
      • Two (2) years of Outpatient medical experience preferred.

 

OTHER SKILLS and ABILITIES: Strong oral and written communication skills required; knowledge of Mohawk Language and traditions preferred. Problem solving skills with the ability to prioritize and manage time effectively.
