Summary: The Internist specializing in Nephrology and Hypertension provides advanced clinical care for patients with chronic kidney disease, acute kidney injury, end‑stage renal disease, electrolyte disorders, and complex or resistant hypertension. This provider delivers comprehensive internal medicine services while offering specialty‑level nephrology care, including diagnostic evaluation, treatment planning, ongoing disease management, and coordination of care with primary care providers, dialysis centers, hospitals, and specialty programs. The role emphasizes culturally sensitive, patient‑centered care rooted in evidence‑based clinical practice, preventive health, and collaborative care models aligned with the needs of the Akwesasne community.
ESSENTIAL DUTIES AND RESPONSIBILITES:
Nephrology Care
- Diagnose and manage chronic kidney disease (CKD stages 1–5).
- Evaluate and manage acute kidney injury and rapidly progressive renal conditions.
- Interpret and manage electrolyte and fluid imbalance disorders.
- Provide consultation and ongoing care for glomerular diseases, nephrotic/nephritic syndromes, and hereditary renal conditions.
- Initiate and coordinate kidney biopsy referrals and follow-up.
- Develop individualized disease‑management plans to prevent CKD progression.
- Provide pre‑dialysis counseling and education.
Hypertension Specialty Care
- Diagnose and manage complex, uncontrolled, or resistant hypertension.
- Evaluate for secondary causes of hypertension.
- Develop and oversee individualized hypertension treatment plans.
- Provide lifestyle modification counseling and collaborate with dietitians and care managers.
Internal Medicine Care
- Provide comprehensive adult primary and specialty internal medicine services.
- Manage chronic diseases such as diabetes, cardiovascular disease, obesity, and metabolic syndrome.
- Perform routine and complex medical assessments, examinations, and medication management.
- Interpret laboratory and diagnostic tests to guide treatment decisions.
Care Coordination & Collaboration
- Collaborate with primary care, cardiology, endocrinology, behavioral health, and other specialty providers.
- Coordinate referrals to dialysis centers, renal transplant programs, and inpatient nephrology services.
- Participate in interdisciplinary case conferences and care planning.
- Communicate and consult with external providers and hospitals to ensure continuity of care.
Patient Education & Counseling
- Educate patients and families on renal disease management, medications, and lifestyle changes.
- Provide culturally sensitive education tailored to the Akwesasne community.
- Promote early detection and self‑management strategies.
Monitoring, Documentation & Compliance
- Maintain accurate, timely, and comprehensive EHR documentation.
- Meet all regulatory, clinical quality, and compliance requirements.
- Follow NYS, IHS, and SRMT clinical protocols and practice standards.
Quality Improvement & Program Support
- Participate in quality improvement initiatives related to CKD, hypertension, and chronic disease outcomes.
- Assist in developing or updating nephrology and hypertension clinical pathways.
- Support population‑health efforts focusing on high‑risk chronic disease groups.
Mandated Reporter: Must become a mandated reporter prior to having contact with children in a job-related capacity and maintain that status throughout employment. This includes completing any necessary training and adhering to all reporting obligations as defined by applicable laws and tribal policies.
SUPERVISORY RESPONSIBILITIES: There are no supervisor requirements for this position.
MINIMUM QUALIFICATIONS:
- Medical degree (MD or DO) from an accredited institution and completion of an accredited Nephrology fellowship; AND
- Completion of an accredited Residency in Internal Medicine and board eligibility or certification in Internal Medicine and active New York State medical license or eligibility to obtain one; AND
- Three (3) years working experience in internal and/or nephrology in a hospital or outpatient setting.
OTHER SKILLS and ABILITIES: BLS and ACLS certifications or obtain within 90 days. Experience in hypertension management and chronic disease care. Strong clinical reasoning, diagnostic, and communication skills. Commitment to culturally respectful care and serving Indigenous communities.
