Summary: The Registered Nurse (Patient Navigator) plays a crucial role in developing and implementing care plans for patients with complex chronic illnesses or significant social determinants of health, guiding them towards their health goals. Additionally, provides navigation support to patients and their families/caregivers, helping them navigate the healthcare system and address any obstacles to receiving care.

 

Essential Duties and RESPONSIBILITIES OF REGISTERED NURSE:

  • Work with the healthcare team to identify any patient with a new significant health diagnosis, or chronically ill patients who may have lost track of follow ups.
  • Act as a liaison between the service providers and advocate for patient in the establishment of a treatment plan that meets their medical, social and cultural needs.
  • Ensure that the patient has all the information they need to make decisions about their healthcare and regularly review the patient's progress toward goals.
  • Identify and determine eligibility of patient for supportive services and resources within the community and assist them in the application process. Contact medical specialty offices to stay informed of supportive services they may offer.
  • Identifying barriers to care experienced by the patient, their families and caregivers in order to guide them through the complexities of the healthcare systems.
  • Assist patients with detailed information about appointments to specialists such as the location of the healthcare facility including the address, parking options, approximate time it takes to get there, proximity to lodging if necessary.
  • Transport and accompany patients to medical appointments if needed.
  • Researches the medical diagnosis using evidence-based sources and conveys complex information to the patient and family in a manner that is easily understood.
  • Provide follow-up care and pre-planning for the next appointment.
  • Explain the function of the health care team members who will implement the plan of treatment to the patient.
  • Work with hospitals on discharge plans for patients.
  • Coordinates with Community Health representatives to provide education in both individual and group sessions to promote patient understanding of various medical conditions.
  • Performs as Staff Nurse when needed
  • Other duties as assigned


MANDATED REPORTER: Must become a mandated reporter prior to having contact with children in a job-related capacity and maintain that status throughout employment. This includes completing any necessary training and adhering to all reporting obligations as defined by applicable laws and tribal policies.


Supervisory Responsibilities:   There are no supervisor requirements for this position.

 

QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • Complete Patient Navigator Training within one year of hire date.
  • Participate in continuing professional development programs as required.

 

Education And/or Experience:

  • Registered Nurse from accredited nursing program in the United States, NYS licensure AND
    • Two (2) years related experience.
    • One-year experience in chronic disease case management PREFERRED

Other Skills and Qualifications: Knowledge of Mohawk language and traditions is an asset. Skilled in the use of computers and related software applications and accessing internet resources. 

 

Reasoning Ability: Ability to provide counseling and emotional support with compassion and respect for the individual/family. 

 

Must have and maintain a valid driver’s license in accordance with the Saint Regis Mohawk Tribe’s Fleet Policy.
