Summary: As an Acute Care Provider, you will provide direct patient care, including diagnosis, treatment planning, and disposition. You will play a key role in advancing acute care, quality initiatives, and patient satisfaction while aligning with program goals and organizational values.
Essential Duties and Responsibilities include the following.
- Patient-centered nurse practitioner caring for the Akwesasne community.
- Provides acute care to patients across the lifespan, from newborns through older adults.
- Orders prescription medications and diagnostic procedures.
- Authorizes and coordinates all specialty and ancillary services.
- Provides timely evaluation and treatment for acute visits, with attention to the needs of patients assigned to the care team.
- Appointments are scheduled in 20-minute slots, with a standard expectation of a minimum of 18 patient encounters per day. The practitioner will remain flexible to accommodate patients, and care team needs and may occasionally see additional patients as clinically appropriate.
- Will participate in a Saturday morning rotation with other staff providers.
- Focus on team-based, patient-centered care.
- Will work closely with RNs, LPN’s, CHR’s, front desk staff, and administrative support staff in addition to provider colleagues.
- Will also collaborate with other patient support services across the organization.
- Correctly and completely documents entries and maintain patient charts in a timely manner as outlined in the Medical Clinic policies and procedures.
- Conducts and documents for employee physical examinations in accordance with Medical Clinic policies and applicable requirements.
- Interact with all colleagues and learners in a supportive and productive manner.
- Remains current in the field of medicine and practices up to date medicine as established by national standards (ACOG, AAFP, CDC, etc.), with practical application based on patient preferences and clinical environment.
- Accurately and promptly provides coding, billing or other financial information to SRMHS billing team and keeps on insurance requests as required.
- Manages multiple tasks simultaneously, including telephone communications, computer-based activities, and in-person visits, while maintaining clear and thoughtful communication with team members and patients.
Mandated Reporter: Must become a mandated reporter prior to having contact with children in a job-related capacity and maintain that status throughout employment. This includes completing any necessary training and adhering to all reporting obligations as defined by applicable laws and tribal policies.
SUPERVISORY RESPONSIBILITIES: This position has no supervisory duties.

Basic Life Support (BLS) must be obtained within 45 days of employment.
EDUCATION AND EXPERIENCE:
- A Graduate of an accredited Nurse Practitioner program with
- Certification in Family Health and New York State licensure as a Nurse Practitioner.
- Two (2) years of advanced clinical practice experience as a licensed Nurse Practitioner, certification in family health or acute care, and current unrestricted New York State licensure as a Nurse Practitioner is preferred.
REASONING ABILITY: The ability to communicate effectively verbally and in writing as appropriate for the audience, so others will understand. Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
