SUMMARY:   Under the direction of the Informational Coordinator, the Caregiver Case Manager is responsible for providing information, assistance, training, support groups, counseling, respite services, and supplemental services to caregivers of older persons (60 years and older), individuals of any age with dementia, and disabled adults.  The Caregiver Support program is designed to support informal caregivers (family, friends, and neighbors) as they carry out their caregiving responsibility. 

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: 

  • Exhibit a friendly and courteous manner when dealing with our employees and fellow associates.
  • Provide services in accordance with Federal, State, and Tribal rules and regulations
  • Maintain client confidentiality and adhere to HIPAA requirements
  • Coordinate and schedule Caregiver support and other support groups
  • Work with individual caregivers on locating, obtaining, and using available services and support
  • Provide counseling to individual caregivers to assist in making decisions and solving problems related to their caregiving roles
  • Facilitate formal education and training programs, classes, or workshops
  • Assess and arrange for respite care and supplemental services
  • Develop and maintain caregiver library
  • Manage Loan Closet to include distribution and return of equipment and maintaining inventory
  • Provide public education on Caregiver Support services to community and outside agencies
  • Identify and provide outreach to individuals needing caregiver support
  • Responsible for obtaining contracts for services and arranging for payment
  • Provide clients with home visits as necessary
  • Attend training as required. To include online and in person.
  • Program reporting to include case notes, program attendance, service units
  • Coordinate with other OFA programs to further program objectives including assisting with Case Management for Nutrition and Home Care programs.
  • Must obtain Case Management certification within 6 months of hire.
  • Other duties as directed by the Informational Coordinator

 

SUPERVISORY RESPONSIBILITIES: There are no supervisor requirements for this position.

 

QUALIFICATION REQUIREMENTS: Must have good verbal and written communication skills.  Interviewing and counseling skills with an ability to work in a team-based environment. Must be able to manage several client cases at once.  Knowledge of social services or human services issues, especially dealing with older adults and family caregivers. Must have reliable transportation. Some evenings and travel required.

 

EDUCATION and/or EXPERIENCE: 

  • Associate degree in Human Services, Social Work, Psychology, Gerontology, Healthcare Administration, or a related field preferred; 

OR

  • High School Diploma/GED with

Three (3) years of experience in healthcare, social services, aging services, case management, or a closely related field.

 

OTHER SKILLS and ABILITIES: Mohawk language is considered an asset.  Must be able to use Microsoft word, email, and online reporting.

 

REASONING ABILITY: Ability to solve problems and prioritize potential solutions. Ability to present unbiased information. Ability to follow oral and written instructions.

 

Must have and maintain a valid driver’s license in accordance with the Saint Regis Mohawk Tribe’s Fleet Policy.
