Position Summary

The Community Health Representative (CHR) serves as a key member of the healthcare team, providing clinical support, patient care coordination, health education, and community outreach services. Working under the supervision of the Nurse Supervisor, CHR assists healthcare providers and nursing staff in delivering efficient, patient-centered care while improving access to health services for individuals and families within the community.
 

The CHR functions similarly to a Medical Assistant by supporting clinical operations, obtaining patient information, assisting with patient flow, coordinating appointments and referrals, reinforcing health education, and promoting wellness initiatives. This position emphasizes compassionate care, cultural sensitivity, patient advocacy, and the integration of community values into healthcare delivery.

 

Mandated Reporter

Must become a mandated reporter prior to having contact with children in a job-related capacity and maintain that status throughout employment. This includes completing any required training and adhering to all reporting obligations as defined by applicable laws and Tribal policies.

 

Key Responsibilities

  • Assist nursing staff and healthcare providers with routine clinical activities and patient care.
  • Obtain and document patient vital signs, height, weight, and other health information as directed.
  • Prepare patients for examinations, procedures, and treatments.
  • Maintain examination rooms, equipment, and supplies to ensure readiness for patient care.
  • Assist with specimen collection, point-of-care testing, and other clinical procedures within scope of training.
  • Perform medication reconciliation and update patient information in the Electronic Health Record (EHR) within the scope of training
  • Provide basic health education and wellness information under the direction of licensed healthcare professionals.
  • Reinforce teaching related to chronic disease management, preventive screenings, immunizations, medication adherence, and healthy lifestyle practices.
  • Participate in community outreach activities, health fairs, wellness events, and educational programs.
  • Build positive relationships with community members to support engagement in healthcare services.
  • Assist in identifying barriers to care and connecting individuals with appropriate resources.
  • Support culturally responsive healthcare practices and community-based health initiatives.
  • Accurately document patient encounters, communications, and services provided in accordance with departmental requirements.
  • Maintain patient records while ensuring confidentiality and compliance with privacy regulations.
  • Assist with data collection, reporting requirements, and quality improvement activities.
  • Answer telephones, relay messages, and provide excellent customer service to patients and visitors.
  • Perform clerical and administrative duties necessary to support clinic operations.
  • Promote respectful, culturally safe, and patient-centered care.
  • Advocate for patient needs and assist individuals in accessing healthcare and support services.

 

Supervisory Responsibilities

Education & Experience

  • High School Diploma or GED required and,

    • Minimum of six (6) months of experience in a medical office, clinic, hospital, public health, community health, or behavioral health setting.

    OR

  • Medical Assistant Certificate, Certification as a Clinical Medical Assistant (CMA), Registered Medical Assistant (RMA), Certified Medical Assistant (AAMA), or equivalent and
    • Three (3) months of experience in a medical office, clinic, hospital, public health, community health, or behavioral health setting preferred.

Other Skills and Abilities

Excellent verbal, written, and interpersonal communication skills. Strong customer service skills and ability to interact professionally with patients, families, staff, and community members.

Ability to manage multiple tasks simultaneously while maintaining accuracy and attention to detail in a fast-paced healthcare environment. Strong organizational and time-management skills. Knowledge of confidentiality requirements and HIPAA/privacy standards. Ability to maintain professional boundaries and cultural sensitivity. Ability to lift and move up to 25 pounds.
